Atrial pacing during a narrow QRS complex tachycardia: what is the mechanism?
نویسنده
چکیده
A 25-year-old woman with a 5-year history of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length, atrial–His-bundle (AH) interval, and His-bundle–ventricular (HV) interval were 750, 60, and 50 msec, respectively. Paroxysmal supraventricular tachycardia with a cycle length of 320 msec was induced by programmed atrial stimulation with a single extrastimulus (Fig. 1). The response to atrial pacing at a cycle length of 260 msec during the tachycardia is shown in Figure 2. The tachycardia was terminated by atrial pacing at a cycle length of 240 msec. Upon termination of the tachycardia, atrial pacing at a cycle length of 260 msec was performed during sinus rhythm (Fig. 3). Ventricular overdrive pacing always terminated the tachycardia. During ventricular pacing, the atrial activation sequence was the same as during tachycardia. What was the effect of atrial pacing on the tachycardia, and what is the mechanism of the tachycardia?

Widening of the QRS complex during atrial pacing: what is the mechanism?
An electrophysiologic procedure was performed in a 50-year-old man with a 20-year history of paroxysmal tachycardia. During sinus rhythm, the atrial-His (AH) interval was 70 msec, the His-ventricular (HV) interval was 45 msec, and the QRS width was 80 msec. During atrial pacing at a cycle length of 290 msec, there was widening of the QRS complex (Fig. 1). The transition from a narrow to wide QR...
متن کاملInteresting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?
Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
متن کاملNarrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?
A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...
متن کاملResponse of narrow QRS tachycardia to premature atrial extra. What is the mechanism?
A 60-year-old woman was referred for radiofrequency catheter ablation of narrow QRS tachycardia that was terminated with intravenous adenosine. Twelve-lead ECG showed no baseline preexcitation. Echocardiogram was essentially normal. The electrophysiological study showed a normal AH interval of 114 ms and HV interval of 48 ms during sinus rhythm. Anterograde study demonstrated no dual AV nodal p...
متن کاملTermination of a narrow QRS-complex tachycardia: what is the mechanism?
A 27-year-old man with a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length was 860 msec, and the atrial-His (AH) and Hisventricular (HV) intervals were 80 and 45 msec, respectively. A narrow QRS-complex tachycardia was induced by atrial overdrive pacing. The cycle length of the tachycardia ...
